These are the requirements you'll be assessed against in the full application. Some have flexibility on a case-by-case basis — discussed in your initial intake call.

Health

Physical and reproductive health baseline.

  • Age 21–40 — clinic and insurance protocols.
  • BMI 18–32 — affects IVF success rates and pregnancy safety.
  • Non-smoker, drug-free for at least 12 months prior to application.
  • Healthy uterine evaluation — confirmed via IVF clinic workup.
  • No major chronic conditions (uncontrolled hypertension, diabetes, autoimmune).

Pregnancy history

Prior pregnancies as a predictor.

  • At least 1 healthy pregnancy and delivery of your own child.
  • No more than 5 prior vaginal deliveries or 3 prior C-sections.
  • Currently parenting at least 1 child in your home.
  • No history of severe pregnancy complications (preeclampsia in 2+ pregnancies, stillbirth, etc.).
  • Last pregnancy delivered more than 6 months ago (recovery period).

Lifestyle and stability

Daily life supports a healthy pregnancy.

  • Stable housing — same residence for 12+ months.
  • Stable household income — not financially dependent on the surrogacy compensation.
  • Partner consent if married or living with a partner — signed acknowledgment required.
  • Reliable transportation to OB appointments throughout pregnancy.
  • Support network (family, friends, community) — emotional support during the journey.

Legal and background

Required for legal parentage and safety.

  • US citizen or permanent resident — for legal parentage and insurance.
  • Reside in a surrogacy-friendly state (California preferred; many other states OK).
  • Pass background check — no felony convictions; no CPS/DCFS open cases.
  • Not currently receiving public assistance (some forms; reviewed case-by-case).
  • Willing to undergo psychological screening — required for ethical compliance.
What disqualifies

Honesty upfront.

We'd rather tell you now than have you spend time on an application that won't move forward. Below are firm disqualifiers we cannot work around.

  • BMI over 32 — IVF clinics require this floor for cycle safety.
  • More than 3 prior C-sections — uterine scarring risk.
  • Smoker or recent drug use — within the last 12 months.
  • Felony conviction — any felony, regardless of how long ago.
  • No prior pregnancy — first-time pregnancy candidates aren't accepted.
  • Open CPS/DCFS case with your own children.
  • Severe untreated mental health condition — flagged in psych screening.
  • Currently pregnant or breastfeeding — recovery period required.

Why we screen so carefully.

Of the surrogate candidates who apply each year, roughly 3% are matched with a family. This isn't gatekeeping for its own sake — surrogacy is medically demanding, emotionally complex, and life-altering for both you and the family. A wrong match hurts everyone.

Our screening protects you: from medical risk you didn't know you had, from a family that isn't right for you, from being approved on day one and discovered unfit on day 90. We say no to most applicants because we say yes for real.
